- What is the best available evidence regarding discharge planning and discharge outcomes for older people discharged from acute care settings to residential care facilities? Discharge planning is the development of a discharge plan prior to the resident/client being discharged from a care facility, with the aim of improving outcomes and minimising costs. The purpose of discharge planning is to facilitate the transition of residents/clients from one level of health care to another. Given the increased number of older residents being admitted to acute settings there have been attempts to accelerate discharging older residents from acute settings and to prevent hospital admission and/or re-admission from residential care settings.
