- Moderate to severe traumatic brain injury (TBI) can lead to a breakdown in spoken discourse abilities with the most common problem being difficulty in having conversations. Communication during everyday activities such as shopping, going to work, studying, meeting new friends, and socializing with old friends can be negatively affected. The impact of these difficulties can be devastating, with a loss of friendships, family breakdown, social isolation, and inability to work. Fortunately, the past 40 years has seen a proliferation of evidence-based communication treatment approaches to improve discourse production following TBI. This chapter explores these treatments with a focus on those with the highest levels of evidence, namely communication partner training approaches and group-based social skills-based treatments. The reader will also gain knowledge of narrative-based interventions, social cognition treatments, project-based treatments, and vocational rehabilitation. Current and future directions with innovative intervention methods and tools are discussed; recommendations for using discourse treatments in clinical contexts are also provided. Further, the chapter offers a practical guide to implementing these treatments using a clinical case as an example and explores other relevant issues to consider on the topic including dosage and timing of intervention and consideration of outcome measures.
