- Objectives: Carotid body tumours are rare in children, and a sclerosing carotid body paraganglioma has never been reported in the paediatric age group. We report a case of a young female with a sclerosing paraganglioma of the right carotid bifurcation and the unique management issues that it created. Methods & results: A nine-year-old girl presented with a painless, solid, right-sided cervical mass, which was encasing the bifurcation of the right common carotid artery. At operation, en-bloc resection of the carotid bifurcation was mandated because of difficult dissection between the tumour and the arterial wall. The external carotid artery stump was oversewn, and a saphenous vein interposition graft was used to reconstruct the common carotid to internal carotid arteries. Final pathology revealed a paraganglioma accompanied with dense stromal sclerosis, consistent with a sclerosing paraganglioma. Conclusion: The diagnosis of a sclerosing carotid body paraganglioma and its treatment in a child is challenging due to its rarity and histological features. We recommend a multidisciplinary approach to such a diagnosis, and plan for the possibility of arterial reconstruction.
