- The issue of patient safety has drawn international attention. The Institute of Medicine (IOM) of the United State of America (USA) commented that health care in the USA is not as safe as it should be - and can be (IOM, 1999). There are many human-made risks existing in the process of providing and receiving healthcare. For example, wrong patient identification, wrong diagnosis, wrong medication, and wrong site of surgery. The harms caused by human errors could be devastating and often fatal. The World Health Assembly (WHA) Resolution WHASS.18 (2002) was a significant international move to urge all countries to pay the closest possible attention to the problem of patient safety. Since then, the World Health Organisation (WHO) has launched many initiatives and programmes to faci1itate actions to enhance patient safety. Examples include the World Alliance for Patient Safety (WHO, 2004) and the comprehensive teaching package for healthcare providers offered by the WHO titled "Patient Safety Workshop: Learning from errors (Donaldson, 2008).
