- Aim: This paper contends that over the last 20 years medical - surgical nursing has changed and should now be considered an international nursing speciality. Unlike other specialities there is no opportunity to become a nurse practitioner in most western countries. These two issues, that is, lack of professional recognition and career development should be addressed. Background: Nearly 30% of practicing nursing work in general medical surgical areas and the majority of patients spend a proportion of their hospital stay is spent in general wards. With the increasing specialization of nursing practice why then is medical surgical nursing not considered a specialty? Currently medical surgical nurses lack a coordinated theoretical approach to care that supports an active engagement and development of the area. A conceptual model for medical surgical nursing will be put forward as a beginning point towards the international recognition of medical surgical nursing as a clinical speciality and the development of nurse practitioners in medical surgical nursing. Conclusion: Active engagement in the medical surgical domain with the use of a conceptual framework as a basis for practice and research will assist in establishing medical surgical nursing as a clinical specialty. When medical surgical nursing becomes a recognized specialty then nurse practitioner becomes a step closer.
