Description: Mr Forsyth is a 42-year-old clerk who has been quite well until recently. He has become somewhat anxious about a vague central chest discomfort that comes on after dinner while sitting watching television and he says that he also gets it between games of squash, which he plays once a week in the evening. He feels that he has cause for concern because his grandfather and uncle died of a stroke and his father and aunt died of a heart attack. The patient has a brother with high blood pressure. Mr Forsyth was examined by his general practitioner who conducted some tests. All seemed normal except for his fasting serum cholesterol level, which was marginally raised at 8.0 mmol/litre (normal: less than 7.2 mmo 1/litre). He was referred for an exercise test.
